The single fitness number that best predicts how long you'll live.
Your peak oxygen uptake is one of the strongest predictors of a long life in all of medicine — and unlike your genes, you can train it. Here's what the science says, and exactly how to raise yours after 40.
Fitness beats almost every other risk factor
If you could take one measurement to forecast your remaining years, VO2 max would be the one to pick.
VO2 max is simply the most oxygen your body can take in and use during all-out effort. Think of it as the size of your aerobic engine: the bigger it is, the more comfortably you climb stairs, carry shopping, chase grandchildren and recover from illness. It sits at the top of the fitness pyramid because everything else — endurance, energy, resilience — is built beneath it.
Here is the part that should get your attention. In 2018, Cleveland Clinic researchers tracked 122,007 adults through treadmill testing and years of follow-up. The least-fit group had roughly five times the risk of dying over the study period compared with the fittest — a gap comparable to, or larger than, smoking, coronary artery disease or diabetes. Crucially, the benefit kept climbing with fitness: the researchers found no upper limit. There was no point at which being fitter stopped helping.
Why does a single number carry so much weight? Because VO2 max is really a measure of how well your whole system works together — heart, lungs, blood vessels and muscle, all delivering and burning oxygen under load. A body that does that well tends to be a body that has stayed resilient in dozens of other ways. That's why it shows up in daily life long before it shows up on a death certificate: it's the difference between strolling up a hill in the Lakes and stopping halfway, or bouncing back from a chest infection in days rather than weeks.
The catch is that VO2 max drifts downwards with age. From your thirties onwards it falls by around 10% per decade if you do nothing — which is how a perfectly capable 45-year-old ends up out of breath at the top of the stairs by 60. The good news, and the whole point of this article, is that training roughly halves that rate of decline, and can claw back years of lost fitness at almost any age. People in their sixties and seventies who take up structured training still make real, measurable gains — it is genuinely never too late to start.
How to improve your VO2 max over 40
You don't need a lab, a coach or a £45-a-month gym membership to move this number. You need two kinds of effort, done consistently: a large amount of easy aerobic work to build the engine, and a small amount of genuinely hard work to raise its ceiling. Sports scientists call this the 80/20 split, and it's what nearly every successful endurance athlete follows. Here's how each piece works in real life.
Zone 2: the easy miles that grow the engine
Zone 2 is steady exercise at roughly 60–70% of your maximum heart rate — a pace where you're working, but could still hold a conversation in full sentences. Brisk uphill walking, easy cycling, swimming or a gentle jog all count.
It feels almost too easy to matter, but this is where your body builds new mitochondria and capillaries — the machinery that actually uses oxygen. Six months of consistent Zone 2 work can lift VO2 max by 10–15% in someone starting from a low base. Aim for three to four sessions of 30–45 minutes a week. A free local parkrun or the NHS Couch to 5K app are ideal on-ramps.
The Norwegian 4×4: the fastest way up
Easy miles build the base, but to push the ceiling higher you need short bursts of hard effort. The best-studied recipe is the Norwegian 4×4: four four-minute intervals at 90–95% of maximum heart rate — hard enough that talking is a struggle — each followed by three minutes of easy recovery. The whole session takes under 40 minutes.
Those four-minute pushes force your heart to pump near its maximum with each beat, which is the specific stimulus that drives VO2 max up. In trials, the 4×4 raised VO2 max by around 7–10% in eight weeks. Once or twice a week is plenty — up a hill, on a bike, or on a rowing machine. This is the 20% of hard work that pays off, provided the other 80% stays genuinely easy.
Strength and steps do the quiet work
VO2 max doesn't live in your lungs alone — it depends on muscle to use the oxygen your heart delivers. After 40 we lose muscle steadily unless we push back, so two short resistance sessions a week (bands, dumbbells or bodyweight) protect the frame that your aerobic engine runs on.
The unglamorous background habit matters too: everyday movement, or the steps, gardening and standing that fill the gaps between workouts. It won't spike your VO2 max on its own, but it keeps you in the game between sessions and makes the hard work stick. You're building a body that stays capable, not just a bigger number on a watch.
A realistic week
You don't have to overhaul your life. Here's roughly what a balanced week looks like for someone in their forties, fifties or beyond — adjust the volume to your starting point and build up slowly.
3× Zone 2
Three easy 30–45 minute sessions — brisk walks, a bike commute, a gentle jog. Conversational pace only.
1–2× Intervals
One or two 4×4 sessions on a hill, bike or rower. Hard four-minute efforts, easy recovery between.
2× Strength
Two short resistance sessions to hold on to muscle — plus a daily walk to tie it all together.
How do you track progress without a lab? A modern Garmin or Apple Watch will estimate VO2 max from your runs and walks for around £200–£300, and the trend line matters more than the exact figure. No wearable? Watch how you feel on a familiar hill, or time yourself over a set distance every few weeks. If it's getting easier, it's working.
The most trainable number in longevity
Of all the health markers you can change, few pay back as generously as the one you build a little at a time, hill by hill.
